Case Manager

Your Case Manager (CM) will facilitate all medical care and will be your ongoing point of contact for any medical conditions while at the Warrior Transition Unit (WTU). The duties and expectations of the CM are as follows:

  • Ensure that the Soldier has received appropriate care.
  • Establish open communications with the Soldier to identify other challenges that the Soldier is experiencing.
  • Use a team approach to promote communication, collaboration and coordination in order to provide Soldier focused care when working with various other entities including the Soldier, Family members, Primary Care Manager (PCM), Physical Evaluation Board Liaison Officer (PEBLO), Social Work Care Manager, Specialists, and the Command. 
  • Document Clinical History
  • Coordinate medical appointments and consults if needed to facilitate treatment of conditions.
  • Provide Soldier Education in the areas of timeliness of care, and treatment of injury.
  • Assist with coordinating care for ongoing follow-up
  • Ensure that your MEB is moving at a reasonable pace. 


Primary Care Manager                

Your Primary Care Manager (PCM) is the primary resource for triad of support. The duties of the PCM include:

  • Develop an integrated treatment plan for each patient
  • Patient Advocacy
  • Provide acute care to Soldiers
  • Improve pain management
  • De-conflict clinical care
  • Collaborate with specialist on post-surgical care
  • Assist with transfers to military and civilian MTFs, VA, CCU, etc.
  • Screen ambulatory patients for TBI and PTSD.
  • Perform initial comprehensive History and Physical Exam
  • Initiate/complete PDHA/PDHRA as needed
  • Follow-up on issues identified on the Post Deployment Health
  • Initiate referrals and communicate with Nurse CM
  • Review entire plan of care with Case Manager, Soldier, and Family
  • Coordination of referrals to Health Net
